A New Tool for Hope

When I first started working in family practice, one thing used to scare me.

Patients would sometimes tell me they were having suicidal thoughts.

Like many providers, my first instinct was simple: keep the patient safe. That usually meant sending them to the emergency room or recommending inpatient care.

Safety always comes first. It always will.

But as I continued my education and later pursued my psychiatric mental health nurse practitioner training, something changed.

I began to understand something very important.

Not every patient who has suicidal thoughts wants to die.

Some people are simply tired, overwhelmed, and stuck in deep emotional pain.

And what they often need most in that moment is not judgment, fear, or immediate hospitalization.

They need a safe place to talk.

They need someone who will listen.

They need a plan.


Creating a Safe Space

As my experience in mental health grew, I became much more comfortable having these conversations with patients.

Instead of feeling afraid, I learned how to slow down and really listen.

Many patients shared things like:

  • I feel stuck.”

  • My brain won’t turn off.”

  • I don’t want to die. I just want the pain to stop.”

Those conversations matter.

When patients feel safe enough to talk openly, we can begin to build a path forward together.

Sometimes that path includes therapy.

Sometimes medication adjustments.

Sometimes lifestyle changes and support.

And sometimes we need new tools.


One of Those Tools Is Spravato

One of the treatments I now offer in my practice is
Spravato.

Spravato is a nasal spray used for people with treatment-resistant depression and for some patients struggling with suicidal thoughts.

What makes Spravato different is how it works in the brain.

Most antidepressants work on serotonin. They can help, but they sometimes take weeks to work.

Spravato works on a different system in the brain called glutamate, which helps brain cells connect and communicate.

For some patients, this can help the brain begin to reset and reconnect, especially when they feel stuck in deep depression.


What Treatment Looks Like

Spravato treatments happen in the office where patients can be monitored safely.

Here is what a typical visit looks like:

  • The medication is given as a nasal spray

  • The treatment takes place in a calm, comfortable space

  • Patients relax while the medication works

  • We monitor them for about two hours

Many patients use this time to simply rest, reflect, or listen to calming music.

The experience can feel different for everyone, but many people describe:

  • Feeling mentally lighter

  • Feeling calm

  • Gaining new perspective

  • Noticing improvement in mood over time

For people living with severe depression or persistent suicidal thoughts, it can feel like nothing will ever change.
